Services include:
· Home assessments for access and safety recommendations in relation to client’s disability/carer’s needs
· Functional capacity assessments
· Minor home modifications (e.g. grab rails)
· Major home modifications (e.g. ramps, wheelchair platform lifts, bathroom modifications)
· Low cost, low risk assistive aids (e.g. shower chairs, toilet aids) assessment and prescription
· Motorised mobility devices (powered wheelchair and mobility scooter) assessment, prescription and training
· Wheelchair (manual and powered) assessment, prescription and training. This includes appropriate cushions and power assists when required.
· Seating assessment and prescription
· Bed assessment and prescription
· Pressure care management
· Personal, domestic and community activities of daily living (ADL) assessment and training
Assessments used can be standardised and non-standardised. Evidence-based interventions are guided by various models of practice such as PEOP model to maximise efficacy of interventions. We work together with clients, support networks and other service providers to ensure actions are followed through and clients' goals are met.
